Conditions: 

1. To confirm your slot, a 50% reservation fee must be deposited at any branch of Philippines National Bank with the following bank details:

Bank Name: PNB Mindanao Avenue Branch
Account Name: SIDEREA HOSPITALITY SOLUTIONS, INC. 
Account Number: 1078-1005-8332
Type of Account: SAVINGS

 2. The remaining balance must be settled on the day itself before the start of the training.

3. For the training to proceed, there must be at least 15 participants. Should we postpone the training under unexpected circumstances, you will be duly notified immediately.

4. In any case that the payment is already made and under unexpected circumstances that you were not able to attend the course, kindly be advised that the payments are non-refundable. Instead, we will reschedule you on the next run or whenever you will be available. Provided that you will be able to notify us seven (7) daysbefore the course starts and an additional payment of Php500.00will be charged for the rescheduling. Furthermore, once we have submitted your name to FoodSHAP®, re-scheduling or any changes will no longer be permitted and payment will be forfeited in case you will not be able to attend.

5. Should we postpone the training under unexpected circumstances, you will be duly notified immediately.

6. Kindly accomplish and send the registration form together with the copy of deposit slip to  [email protected].

 

Who Should Attend:

All Food Handlers working in the catering or retail setting where food is prepared, cooked, and handled. Particularly relevant to environments such as pubs, hotels, restaurants, supermarkets, retail environments, hospitals, care homes, schools, prisons, or any establishment that handles and serves food.

 

Assessment:

The course shall be assessed through written and practical exams. Candidates must pass both assessments to receive their certificates from FoodSHAP®. Furthermore, students who fail the practical or written exam at any level may retake the exam for free up to 3 times, after which they need to re-attend the training. Students must pass both written & practical summative assessment to be eligible to receive their FoodSHAP® FSTES.
